What to Bring to Your Exam
- Your insurance card and photo identification.
- Please verify your insurance benefits with your insurance company before your appointment.
- Why? We do provide this service as a courtesy to our patients, but it is ultimately your responsibility to verify your benefits with your respective insurance company. If you have questions, we are here to assist.
- A list of all medications that you are currently taking and a list of your medical history.
- Why? There are many systemic/OTC medications that have side effects which can impact your vision and eye health. Knowing these medications can help our doctors in providing a more detailed and thorough examination. There are many systemic conditions (diabetes, thyroid disorder, auto-immune diseases, etc.) that can have an impact on your vision and eye health. Telling your eye doctor about these conditions allows us to provide a more detailed examination.
- Glasses and/or sunglasses you usually wear, even if you typically wear contact lenses.
- Why? This allows us to make sure your glasses prescription is current and allows us to provide a more accurate prescription.
- If you wear contact lenses, wear them to your exam and bring a written copy of your current prescription or the packaging your lenses came in.
- Why? This allows us to verify the contact lenses that you are most currently wearing and allows us to provide a more accurate prescription.
